Do you have US citizenship?

The DAFT route is only open to US citizens. Japanese nationals have their own treaty route with the same rules.

Is your US passport valid for the whole application period?
Have you chosen a business structure?

Most DAFT applicants start as a sole proprietorship (eenmanszaak); some choose a BV.

Do you have at least €4,500 of your own money for the business?

It has to be your own capital. Borrowed money doesn't count, because the business then also owes it.

Is your business registered with the KVK (Chamber of Commerce)?
Do you have a Dutch business bank account with the €4,500 in it?
Do you have an opening balance sheet that shows the €4,500 as equity?

A bank statement shows a balance, not equity. The opening balance sheet is the document that proves the investment.
